This will provide the foundation to ensure they are able to achieve the best opportunities and keep as healthy and well as possible throughout their lives.
We want to ensure that families, individuals and communities are thriving and resilient with access to good jobs, affordable housing, leisure activities, lifelong training, education and learning, health and care services are are able to enjoy the place in which they live.
We want to ensure that people on the Isle of Wight are able to live independently in their own homes with appropriate care support. We want to make sure older residents are supported to play an active role in their communities and supported to maintain and develop their social and community networks.
The Living Well approach provided by Age UK Isle of Wight supports people aged 18 or over who may have low level needs requiring support or advice but who do not meet eligibility criteria to receive adult social care services, to lead fulfilling lives and maintain independence. The approach seeks to provide personalised and coordinated services to help you get the right support, at the right place at the right time.
Everyone's idea of living well will be individual to them; it will depend on your general health, your current circumstances and your own health goals.
The Living Well Team will work with you to develop a personal action plan to help you achieve your goals. Their approach also includes specialised support for older people, people living with learning disabilities, people living with metnal health conditions and carers.
