We are working to ensure that children on the Isle of Wight are supported to get the best start in life that will lead to good health and wellbeing. This will provide the foundation to ensure they are able to achieve the best opportunities and keep as healthy and well as possible throughout their lives.
We want to ensure that families, individuals and communities are thriving and resilient with access to good jobs, affordable housing, leisure activities, lifelong training, education and learning, health and care services are are able to enjoy the place in which they live.
We want to ensure that people on the Isle of Wight are able to live independently in their own homes with appropriate care support. We want to make sure older residents are supported to play an active role in their communities and supported to maintain and develop their social and community networks.
Mr X, an elderly widower living alone, suffered from a number of long term illnesses which in turn made this naturally happy and confident person, over weight, lonely and depressed.
This once jovial person lost the will to participate in life. He no longer looked after himself or cleaned his home and it had become a health hazard and fallen into dangerous disrepair.
What was put in place to help:
The Care Navigator used all the services at their disposal to help this man regain control of his life. For his home, they arranged for an Attendance Allowance, an initial deep clean of the house followed by a regular cleaner, along with replacement of essential white goods. For himself, they arranged for some comfortable, appropriately fitting shoes, access to a nail cutting service, introduction to social clubs and healthy eating services.
How things are working now:
The change in Mr X has been noticeable. He is more upbeat, happier and healthier. He is committed to a routine of looking after himself and his home and using the new items he has been given.
