We are working to ensure that children on the Isle of Wight are supported to get the best start in life that will lead to good health and wellbeing. We want to ensure that families, individuals and communities are thriving and resilient with access to good jobs, affordable housing, leisure activities, lifelong training, education and learning, health and care services are are able to enjoy the place in which they live.
We want to ensure that people on the Isle of Wight are able to live independently in their own homes with appropriate care support. We want to make sure older residents are supported to play an active role in their communities and supported to maintain and develop their social and community networks.
Elderly Mrs E was having some memory problems and not coping well at home and needed some support.
Recuperating at home after an illness, Mrs E started to experience memory problems which led to potentially dangerous situations such as forgetting food cooking in the oven. As a result she found herself forced to live on less nourishing, cold food and struggled to regain her health and confidence.
What was put in place to help:
Finding that there was no provision for delivery of hot meals to her remote location, the Care Navigator came up with the perfect solution with the local pub. They arranged for delicious and nourishing hot meals to be delivered to her every day, and after 2 weeks of eating properly, she regained her strength and was less confused.
Furthermore, the Care Navigator replaced the batteries in her smoke alarm and helped her to apply for Attendance Allowance with which she was able to employ a regular cleaner.
How things are working now:
The difference a hot meal and a clean and safer home have made to her is incredible! She is no longer frail and helpless. She has gained strength and confidence and has been able to invite friends over so she feels less isolated, independent and much happier.
